	I would like to request a key to the regular practice rooms.
	I would like to requst a key to the Grand Piano practice room (Piano Majors only).

Reason	Keys for anything other than a practice room must be approved by your major professor.
	Your key paperwork will be processed within one week of your submission of this request. If your request is approved your paperwork will be delivered to the Music Office. You must then pick your paperwork up and take it to the Key Desk with your CATCARD in order to pick up your key. The Key Desk is only open on Monday - Friday from 9am - 3pm, and your paperwork is only good for 1 month from the date typed on it. Please do not delay in picking up your paperwork.
	I understand that the use of a practice room or other rooms in the School of Music is a privilege granted by the University of Arizona for educational purposes. I understand that I may not use any part of the Music Building (practice rooms, classrooms, studios, rehearsal rooms, etc.) for the purpose of giving music lessons or instruction for pay. If I violate this policy I understand that I will be subject to Disciplinary Action. By submitting this request I agree to these conditions.
